You must complete all FORMS  below

to be considered for the C.N.A. Medication Aide Course starting March 4, 2024

We must have the following information

NO LATER THAN  December 29, 2023 

 FOR  THE CLASS TO START ON TIME!!!!!! 

  • ​

  •  2 References (must be from a R.N) 

  •  

  • a letter from your employer verifying  Hours worked in the last two year (must have worked at least 1080 hours in the last 2 years)

  • YOU MUST TAKE AN ENGLISH TEST AT ORONO ADULT EDUCATION

tHE RIVERSIDE APPLICATION CAN BE COMPLETED AT ORONO ADULT EDUCATION WHEN YOU TAKE YOUR ENGLISH TEST.

                                 OTHER IMPORTANT INFORMATION

Arrange for payment of class with Riverside Adult Education. You may need to pay privately or your employer may provide financial support.

Discuss the class schedule with your employer to assure that you will have the time off that is necessary for participation.

Verify that your employer is willing and able to provide the necessary 40 hours of clinical that includes direct supervision by a nurse.
